Save time during your appointment by filling out your registration forms in advance through our online portal.
If your child is under the age of 18, a parent or legal guardian must accompany him or her to the first appointment. Children who arrive without a parent or legal guardian present will have their appointments rescheduled.
Appointment Deposit Policy
For cosmetic appointments, we may require a deposit in advance. Medical appointments do not require a deposit. We’ll offer you more information when you schedule your appointment.
Missed Appointment & Cancellation Policy
We realize that unplanned issues happen to all of us. However, we respectfully request that you cancel your scheduled appointment via phone or email at least 24 hours in advance.
If you do not cancel ahead of time, you will be charged an $85 missed appointment fee, and you may forfeit your deposit. This fee is not covered by private insurance or Medicare, and it will be your responsibility to pay at the time of your next visit. Our aim is to open otherwise unused appointments to other patients, not to collect fees. We greatly appreciate your cooperation and consideration.
As a courtesy to our clients, we will send an appointment reminder via text and email 3 days prior to your scheduled visit. We added text and email reminders to offer you a higher level of service and a more efficient means of communication.Back to Top
